Registration Forms allow you to collect additional information from your ticket purchasers. Whether you're interested in learning how they discovered your event or need more specific information such as their food choice for a dinner event, Registration Forms allow you to prompt ticket buyers for a response to each question once per ticket or once per order and also give you the option to ask specific questions for specific ticket types.
Create a Registration Form for an Event
Navigate to the event by selecting the Events tab in the main navigation and then selecting the event from your Event List. If you are on the event Dashboard then select the blue button that says "Edit". From the Edit page, scroll to the bottom and click on the "Show Advanced Settings" link to expand the advanced settings.

In the advanced settings is the option to create a Form. Click the "Create a Form" button on the Registration Forms card to open the form configuration window.

Name your registration form and specify whether the questions within the form will be asked once per order or once per ticket.
Tip: Selecting to collect information "Once per ticket" is often used to collect the name of each individual attendee.

+Add a Custom Question
Click the +Add a Custom Question. In the window that appears, type the question you'd like to ask your ticket buyers. Specify whether or not the question will be required using the Required toggle and then choose the type of response you'd like to collect.
- Text - Open text field for ticket buyer to enter written response
- Radio Button - Ticket buyer selects a single option from a list
- Checkboxes - Ticket buyer selects all options that apply from a list

Restrict to Specific Ticket Types
If you would only like to ask the questions in your registration form when specific tickets are purchased, click the toggle labeled Restrict to Specific Ticket Types at the bottom of the form card . Any tickets that you have created for this event will appear by group. Select the checkbox next to the group(s) you'd like to prompt the form for.
Toggle It On
At this time, only one form can be added to an event. Because of this, it is not possible to ask different questions for different ticket types. Rather, you can ask that the one form be presented to all or some of the ticket types.
Click the Save Form button to save your registration form. When your registration form is saved, all ticket purchases for the event going forward will include the form. If you need to turn your registration form off at any point, click the toggle on the Registration Form card.

Edit an Existing Registration Form
When you have an existing registration form you can add or remove and make edits to questions. To edit the registration form you will need to navigate to the edit page for your event and select the "Show Advanced Settings" option. In the advanced settings, you will see the name of any existing form in the Registration Forms section. Click on the name of the form to make any changes to the existing form.
